Step 1-  Visit the official website portal, i.e. ndhm.gov.in

Step 2. Click on the “Create Your Health ID Card Now” option at healthid.ndhm.gov.in

Step 3- There are 2 Options to Create Health ID by Aadhaar Number or Mobile Number

Step 4- Now, you need to enter your Aadhar card number, OTP and fill in the captcha code

Step 5- After submitting the captcha, Enter Mobile number and OTP, check your details properly

Step 6- After successful submission, you can create a username

Step 7- Once you have created your username, the Health id card will be generated with the help of the identification proof

Step 8- At last, click on the Download ID button.

How to create NDHM Card?

To create NDHM card you will have to login to healthid ndhm gov portal. Just open any browser and enter the query "healthid ndhm gov portal" or "national digital health card apply online" and the search result will direct you NDHM.gov.in, where you can fill up the online application and submit the details.
